Incident Investigation and Root Cause Analysis (IIRCA)

 Who should enroll: Safety practitioners who may have some investigation training, but wish to learn more about investigation techniques and root cause analysis. Supervisors who must oversee investigations and corrective actions. Managers who must review incident investigations reports. Anyone looking to gain an understanding of how investigations should be conducted and how they should fit into the company Health and Safety System or Strategy.

The course: Part one covers investigation and reporting, most common loss causation models, and six phases of incident investigation. Attendees will learn to effectively investigate incidents and generate necessary reports. Supervisors and managers will gain insight into the process to allow them to effectively review investigation reports to determine their validity.

Part two will cover root cause analysis utilizing the most commonly used loss causation model (Bird/DNV) and the associated Systematic Cause Analysis Technique.

Attendees will learn the theory and through practical exercises, based on real incidents. The course will deliver a working knowledge of incident causation and root cause analysis.

Incident Mirror Technique

 This workshop is invaluable for those who must review investigation reports to ensure they are valid and meet requirements. Not everyone is an experienced investigator or even a trained investigator, yet there may be a requirement for managers, supervisors, and even safety committee members to review incident reports. 


The incident mirror technique allows people with limited training to evaluate investigation reports to help ensure the company is gaining the maximum value from this activity.


The workshop is also valuable for those who complete investigation reports as the techniques taught help them check their work and improve the quality of their reports.


Actual incidents can be integrated into the training to make it more relevant for employees, and the overall result of this seminar is improved quality in reports and better chances that incident investigations find the underlying cause or the root cause of incidents.


It is the failure to find root causes or underlying causes that leads to future incidents. This session can be combined with Incident investigation training
